Referral

Referrals are accepted from parents and professionals. Following referral, I offer one-off assessments for individuals in their own environments, or work with you to meet the needs of the individual, for example, to provide intervention to address any identified difficulties which can include training for carers and families if needed.

Please fill in the referral form, and email or post back to me - my contact details are on the form. Please provide information by answering all the questions, but add further information that you think would be helpful.
